EFFECT OF MULTIMEDIA EDUCATION ON IMPROVING FAMILY KNOWLEDGE, ATTITUDES, AND PREVENTIVE BEHAVIORS FOR STROKE PREVENTION IN CIPAYUNG, EAST JAKARTA

Abstract

Stroke is a leading cause of death and disability; thus, family-based prevention is essential, especially for families with members with hypertension. This study assessed the association between an animated-video multimedia education intervention and changes in family knowledge, attitudes, and preventive behaviors regarding stroke. A quasi-experimental one-group pretest–posttest design without a control group was implemented in Lubang Buaya Subdistrict, Cipayung District, East Jakarta, Indonesia, from June to November 2025. Thirty families were recruited using purposive sampling. Data were collected using validated questionnaires on stroke-prevention knowledge, attitudes, and behaviors administered before and after the intervention, and analyzed with paired t-tests. Knowledge scores increased from 8.73±1.34 to 11.57±1.19 (mean difference 2.84; 95% CI 2.28–3.39; p<0.001), attitude scores from 19.30±3.55 to 23.57±2.50 (mean difference 4.27; 95% CI 3.51–5.03; p<0.001), and behavior scores from 6.10±1.00 to 7.90±0.80 (mean difference 1.80; 95% CI 1.38–2.22; p<0.001). The findings suggest that animated-video multimedia education is associated with improved family-level stroke prevention outcomes and may be integrated into promotive–preventive strategies in primary care. Because knowledge was assessed immediately after education, changes primarily reflect very short-term (working memory) retention; follow-up assessments are needed to evaluate long-term retention.

Abstrak
Stroke merupakan penyebab utama kematian dan disabilitas, sehingga pencegahan berbasis keluarga menjadi penting, terutama pada keluarga dengan anggota yang memiliki riwayat hipertensi. Penelitian ini bertujuan menganalisis hubungan intervensi edukasi multimedia (video animasi) dengan peningkatan pengetahuan, sikap, dan perilaku keluarga dalam pencegahan stroke. Penelitian menggunakan desain kuasi-eksperimental one-group pretest–posttest tanpa kelompok kontrol, dilaksanakan di Kelurahan Lubang Buaya, Kecamatan Cipayung, Jakarta Timur pada Juni–November 2025. Sebanyak 30 keluarga direkrut melalui purposive sampling. Data dikumpulkan menggunakan kuesioner pengetahuan, sikap, dan perilaku pencegahan stroke yang telah divalidasi, diberikan sebelum dan sesudah intervensi. Analisis dilakukan menggunakan uji t berpasangan. Hasil menunjukkan peningkatan bermakna pada seluruh variabel. Rerata skor pengetahuan meningkat dari 8,73±1,34 menjadi 11,57±1,19 (selisih 2,84; 95%CI 2,28–3,39; p<0,001), sikap meningkat dari 19,30±3,55 menjadi 23,57±2,50 (selisih 4,27; 95%CI 3,51–5,03; p<0,001), dan perilaku meningkat dari 6,10±1,00 menjadi 7,90±0,80 (selisih 1,80; 95%CI 1,38–2,22; p<0,001). Temuan ini menunjukkan edukasi multimedia berbasis video animasi berhubungan dengan perbaikan pengetahuan, sikap, dan perilaku keluarga dalam pencegahan stroke dan berpotensi diintegrasikan sebagai strategi promotif–preventif di layanan kesehatan primer. Karena pengetahuan diukur melalui post-test segera setelah edukasi, perubahan skor terutama merefleksikan ingatan jangka sangat pendek (working memory level). Evaluasi lanjutan diperlukan untuk menilai retensi jangka panjang.
