PROFIL ANEMIA DAN TROMBOSITOPENIA PADA PASIEN SIROSIS HEPATIS NON HEPATITIS
PROFILE OF ANEMIA AND THROMBOCYTOPENIA IN PATIENTS WITH NON-HEPATITIS LIVER CIRRHOSIS
Abstract
Anemia is a common hematological complication in liver cirrhosis, caused by gastrointestinal bleeding, hypersplenism, and bone marrow dysfunction. This study aimed to analyze the relationship between hematological parameters and the severity of non-hepatitis liver cirrhosis. A cross-sectional observational study was conducted on 30 patients with non-hepatitis liver cirrhosis at Dr. Ferdinand Lumbantobing General Hospital, Sibolga, from December 2023 to August 2024. Laboratory parameters included hemoglobin, MCV, MCHC, and platelet count, analyzed using the Spearman correlation test (p<0.05). Among 30 patients, 21 (70%) were male and 9 (30%) female. Macrocytic anemia was predominant (53.3%), followed by microcytic (26.7%) and normocytic (20%). Most patients (50%) were in Child-Pugh B class, and thrombocytopenia occurred in 70% of cases. Significant correlations were found between hemoglobin, MCV, MCHC, and platelet count with cirrhosis severity (p=0.00). These findings indicate that anemia, particularly macrocytic type, is closely linked to chronic liver disease progression. Hematological parameters may serve as simple and valuable clinical indicators for assessing disease severity. A significant relationship exists between anemia and the severity of non-hepatitis liver cirrhosis.
Abstrak
Anemia merupakan komplikasi hematologis yang sering dijumpai pada pasien sirosis hepatis, disebabkan oleh perdarahan saluran cerna, hipersplenisme, serta gangguan fungsi sumsum tulang. Penelitian ini bertujuan menganalisis hubungan antara parameter hematologi dan derajat keparahan sirosis hepatis non-hepatitis. Penelitian observasional dengan desain potong lintang ini melibatkan 30 pasien sirosis hepatis non-hepatitis di RSU Dr. Ferdinand Lumbantobing, Sibolga, periode Desember 2023-Agustus 2024. Data laboratorium meliputi kadar hemoglobin, MCV, MCHC, dan jumlah trombosit. Analisis menggunakan uji korelasi Spearman dengan tingkat signifikansi p<0,05. Dari total 30 pasien, 21 (70%) berjenis kelamin laki-laki dan 9 (30%) perempuan. Jenis anemia yang dominan adalah makrositik (53,3%), diikuti mikrositik (26,7%) dan normositik (20%). Sebagian besar pasien (50%) berada pada kategori sirosis sedang (Child-Pugh B), dan 70% mengalami trombositopenia. Terdapat hubungan signifikan antara hemoglobin, MCV, MCHC, dan trombosit dengan derajat keparahan sirosis hepatis (p=0,00).b Hasil ini menunjukkan bahwa anemia, khususnya makrositik, sering berkaitan dengan progresivitas penyakit hati kronis. Parameter hematologi berpotensi menjadi indikator klinis sederhana dalam menilai keparahan sirosis. Terdapat hubungan bermakna antara anemia dan derajat keparahan sirosis hepatis non-hepatitis. Parameter hematologi dapat digunakan sebagai indikator progresivitas penyakit hati kronis.
