ANALISIS ELEKTROKARDIOGRAFI SERIAL PADA KASUS WELLENS SYNDROME TIPE B
SERIAL ELECTROCARDIOGRAPHIC ANALYSIS IN A CASE OF TYPE B WELLENS SYNDROME
Abstract
Wellens syndrome merupakan pola elektrokardiografi khas yang mengindikasikan adanya stenosis kritis pada arteri left anterior descending (LAD) dan berisiko tinggi berkembang menjadi infark miokard anterior luas. Kondisi ini sering tidak disertai elevasi segmen ST sehingga dapat terlewatkan dalam praktik klinis. Publikasi kasus Wellens syndrome masih relatif jarang dilaporkan di Indonesia, padahal pengenalan dini terhadap pola elektrokardiografi khas ini sangat penting untuk mencegah keterlambatan diagnosis dan terjadinya kerusakan miokard yang lebih luas. Kami melaporkan seorang laki-laki usia 59 tahun dengan keluhan nyeri dada kiri yang memberat, menjalar ke punggung, dan disertai keringat dingin. Pasien memiliki faktor risiko berupa merokok berat, diabetes melitus tidak terkontrol, dan aktivitas fisik rendah. Pemeriksaan elektrokardiografi serial menunjukkan inversi gelombang T pada lead anteroseptal khas untuk Wellens syndrome tipe B. Peningkatan troponin membuat diagnosis NSTEMI dengan Wellens syndrome ditegakkan. Pasien mendapatkan terapi optimal sesuai pedoman berupa antiplatelet ganda, antikoagulan, statin, beta blocker, dan nitrat dengan perbaikan klinis. Kasus ini menegaskan pentingnya pengenalan dini Wellens syndrome melalui interpretasi EKG yang cermat untuk mencegah terjadinya infark miokard anterior luas melalui intervensi yang tepat waktu.
