ADVANCED LUNG CANCER INFLAMMATION INDEX AS A PREDICTOR OF MORTALITY IN ADVANCED STAGE LUNG CANCER
Abstract
Peran inflamasi sistemik dan status nutrisi semakin diakui sebagai penentu prognosis. Advanced Lung Cancer Inflammation Index (ALI) dilaporkan berkaitan dengan mortalitas pasien kanker paru. Data mengenai hubungan ALI dengan mortalitas pasien kanker paru stadium lanjut di Indonesia masih terbatas, sehingga penelitian ini penting untuk mengevaluasi nilai prognostik ALI. Penelitian ini merupakan studi observasional analitik dengan desain kohort retrospektif yang dilakukan di RS Mohammad Hoesin Palembang. Sampel diambil secara consecutive sampling dari pasien kanker paru stadium lanjut yang terdiagnosis tahun 2022–2024 dan memenuhi kriteria inklusi. Nilai ALI dihitung dari data awal pasien. Mortalitas dievaluasi melalui rekam medis dan follow up enam bulan. Analisis data dilakukan menggunakan uji statistik bivariat dan kurva Kaplan–Meier. Sebanyak 419 pasien kanker paru stadium lanjut dianalisis, dengan mortalitas enam bulan sebesar 53,2%. Median skor ALI pada kelompok meninggal lebih rendah dibandingkan kelompok hidup (14,7 vs 25,1; p<0,001). ALI rendah berhubungan signifikan dengan peningkatan risiko kematian (aHR 5,15; IK95% 1,92–13,85). Analisis Kaplan–Meier menunjukkan median kesintasan 12 minggu pada ALI rendah (p<0,001). Skor ALI memiliki kemampuan prediktif rendah–sedang. Skor ALI rendah berhubungan signifikan dengan peningkatan mortalitas dan penurunan kesintasan pasien kanker paru stadium lanjut.
